PTW Job Description
Permit To Work (PTW) refers to management systems used to ensure that work is done safely and efficiently. These are used in hazardous industries and involve procedures to request, review, authorise, document and most importantly, de-conflict tasks to be carried out by front line workers. Permit to work is an essential part of the integrated management of business critical maintenance processes. Control of work is made up of permit to work, hazard identification and (RA), and isolation management (IM).
A permit to work is not a replacement for robust risk assessment, but can help provide context for the risk of work to be done. Studies by the UK Health and Safety Executive have shown that the most significant cause of the maintenance related accidents in the UK chemical industry was a failure to implement of effective permit to work systems. Common failures in control of work systems are a failure to follow the permit to work or isolation management procedures, risk assessments that are not suitable and sufficient to identify the risks, and/or the control measures and a combination of the two.
Responsibilities Of Permit Receiver
Initiating and completing all applicable section of the .
Supervising the work prescribe in the permit to work to ensure it is conducted in the most efficient manner.
Notifying the issuer of work progress and completion, and ensuring that the site is returned to a safe and operational state .
Taking adequate care to preserve the permit in good condition .
5 . Providing and arranging all necessary resources related with performing the work, including standby- man for
of work permit is required.
Responsibilities Of Permit Issuer
Reviewing, endorsing and issuing the permit to work. For all work permit application, site visit is mandatory to enable evaluation and specification of safeguards to avoid .
Completing the relevant sections of the permit to work.
Ensuring the participation in the permit to work carefully and conduct work in accordance with the permits requirements.
Initial test before issuing the permit if required.
Providing shift revalidation and conducting gas testing to ensure that work is safe to proceed.
Signing off permit that required work has been completed.
Participation in joint annual of permit to work files.
